Our initial home assessment is conducted at no cost and is obligation free.

This comprehensive assessment looks at each client’s lifestyle, medical condition and the kind of support that is required to enable them to continue living independently at home.
We appreciate that the first assessment or meeting can occur during a difficult or stressful time for our clients and their families.

You will have questions to ask and important decisions to make, and it is our responsibility to ensure you are provided with all the information you need so you can make the most appropriate and safe care choice.
We are here to answer your questions and offer you the benefit of our extensive experience without pressure and at no obligation.

During our assessment we will discuss with you:

•       Your care requirements
•       Health and medication
•       The days and times that care would be most beneficial
•       The type of carer you would prefer
•       Emergency contacts and response
•       The type of services we provide, their costs and options for savings
•       Goal setting
•       A tailored care and wellness plan

Following assessment, we will provide a quote outlining the type, hours and costs of services.
Chiltern Home Care will then develop a care and wellness plan individually tailored and responding to specifically identified needs, problems and goals.

Carer Selection
We will select the carer most suited to meet the client’s needs and preferences and if required, arrange a personal introduction.

Service Delivery
The services you require will be delivered on the days and at the times most convenient to you.

24 Hour Support
All clients benefit from our 24-hour support service, which allows for peace of mind should unexpected needs arise.

Our Rates
Chiltern Home Care gives you the flexibility to choose the care options that best suit your individual needs and budget.
We offer excellent value for money and our prices are very competitive.
Our services are charged on an hourly basis and rates for longer-term care may be negotiated.

Packages range from a one-hour service through to 24-hour care, Monday to Sunday, including public holidays.
